How to Claim
Columbus is committed to assisting you if you need to make a claim on your income protection policy.
You must provide notice of a claim by telephoning the administrator of your policy, MMS on: 01132 558 611. For written enquiries:
Claims Department, MMS, Melbourne House, Melbourne Street, Farsley, Pudsey, Leeds, LS28 5BT
MMS will send you out a claim form. Please complete this as soon as reasonably possible, providing all the information asked for to enable your claim to be processed. MMS will provide full details of the supporting documentation required to process your claim.
